Health Link Team
The Health Link Team is a social work service funded by the
Royal Borough of Kensington and Chelsea. It works as part of a
multi-disciplinary team within Chelsea and Westminster hospital
What we do
The team provides a social work service to:
- children receiving hospital treatment (including outpatients)
and attending the Accident and Emergency Department
- children attending the Cheyne Child Development Service
- women who are expecting a baby or have just given birth, and
parents with a baby on the Neonatal Intensive Care Unit
- women who are expecting a baby and who are booked with the
Community Midwives Team in South Kensington and Chelsea
- families visited by Health Visitors in South Kensington and
Chelsea.
The range of difficulties the social worker could help you with
can include:
- personal and family relationship problems
- emotional and practical problems
- issues regarding disabilities and illness
The Health Link Team can also provide general advice on
benefits, homelessness, fostering, adoption, and alcohol and drug
use.
The team also have a responsibility for the investigation of
concerns about children and vulnerable adults.
The Team includes Senior social workers, social workers, and
administrator.
How we work
When making plans to help you and your family, the team will
seek your views and you will normally be involved in meetings where
people are considering the best way to offer a service.
The team will respond to the needs of you and your family in a
way that respects individual and cultural differences and
identity.
Who can get help?
The service is for the residents of Kensington and Chelsea. If
you live outside the borough and you or your child are receiving
inpatient treatment, the team will help with your immediate problem
and put you in touch with your local Family Services office.
For inpatients, we try to respond to requests for social work
help on the same day.
Making contact
The Social Work Department operates from 9am to 5pm, Monday to
Fridays.
You can telephone the social worker attached to your ward or
clinic yourself, or you can ask a doctor, nurse or midwife to make
an appointment on your behalf. If the social worker attached to
your ward or clinic is not available, a duty social worker can be
contacted on 020 8846 1316. Messages can be left with the team
administrator.
