Advice on staying healthy as you grow into an adult
This is your go-to resource for all things health and wellbeing. Some of the services that have helped you so far may stop at age 16 or 18. Therefore, it is important to plan how health needs will be met, and by whom, in the future. This page is designed specifically for young adults aged 16-25 with special educational needs or disabilities (SEND) in Westminster/Kensington & Chelsea. General Health Information
Need advice or medical help
For all medical emergencies, call 999 immediately. For non-emergency medical advice, you can contact NHS 111. This service is available 24 hours a day, 7 days a week, and is helpful if you can’t wait for a GP appointment.

Registering with a GP
As you transition to adult healthcare, it is important to be registered with a general practitioner (GP). To find and register with a GP near you, visit the NHS Find a GP tool.
GPs can support you with everyday health concerns, prescribe treatments, and refer you to specialist services if needed. After any specialist treatment, your GP will continue to provide your ongoing care. If further support is required in the future, your GP can make another referral.
District nurses can visit you at home to help with certain medical needs and health interventions. You may also receive care from specialist services in hospital if required.
Reasonable Adjustments
Health services should make reasonable adjustments to meet your needs. You can speak to your GP or service provider about what changes can be made for you.
NHS Prescriptions
Some young people with SEND are eligible for free prescriptions. Check your eligibility here.
Dental Care
You should continue to access dental services regularly. Many community dental clinics are experienced in working with people with additional needs. Find a dental service near you.
Learning Disabilities Support
- Annual Health Checks (14+): Talk about your health, pick up issues early, and get a personalised Health Action Plan
- Learning Disability Register: Gives access to annual health checks and reasonable adjustments
- Vaccinations: Free flu and COVID-19 boosters for those on the LD Register
General Wellbeing and Self-Care
