Changing your surname in your passport

You can apply for a passport in your new name up to 3 months before your marriage or civil partnership ceremony. Find out more on GOV.UK and download a PD2 form

The completed form then needs to be signed by the registrar. 

Drop by at our reception at Chelsea Old Town Hall during our opening hours:

  • Monday to Friday, 9am to 4pm

There is a £10 fee for this service.
