(You are invited to name up to six schools on the form. You are applying for schools at a time where places have already been allocated, so it is not always possible to offer your child a place at a school that you may prefer or is closest to your home. All schools have a published admission limit that they must not exceed, so unless the school has a vacancy, you can request that your child’s name be placed on a waiting list. If you are applying for schools outside of the Royal Borough, you must also use this form. If you are applying for more than one child, it will often be the case that a place can be offered for one child but not the another because the year group applied for is full. There is no guarantee that all children in one family can be offered a place, so this must be taken into account if you find yourself in this situation. If it is not possible to offer your child a place at any of the schools you have requested, you will be provided with the details of schools with available places)
If there is a sibling (brother or sister) attending one of your preferences, please indicate the name of the school, full name and date of birth of the child below. The children must be living at the same address to be considered as a sibling.
