Completing your ceremony choices form
After you’ve given your notice of intention and have paid your final ceremony attendance fee, your final step will be to submit to us your choices for your ceremony. To do this, please email [email protected] to request your ceremony choices form. Please include your full names, and the date, time and location of your ceremony in the request.
