Preparing for Transition and Adulthood Team 14+

The Bi-Borough Preparing for Transition and Adulthood Team for Disabled Children aged 14+.

This team will support and prepare you and your young person to navigate through the transition period. This could include a number of options such as referring onto adult social care, support plans to meet outcomes focused on independence through life skills training. Practitioners in this team will be able to work with the young people looking towards the future and thinking of options around education, employment and training.

This team will be working closely with in-house operations to deliver programmes directly supporting the development of practical skills that will promote independence.
