Change4Life Service

We are delighted to introduce the Bi-Borough Change4Life Service for Kensington and Chelsea and Westminster.  The service aims to ensure that children, young people and families across Kensington and Chelsea and Westminster are supported to be healthy and happy in the communities where they live, learn and play.

The Change4Life service offers free universal Change4Life clubs, one-to-one coaching, workforce training and capacity building and neighbourhood projects.

Please download the leaflets for more information about the service offer and dates of our upcoming Change4Life Clubs for expectant parents, children aged 0-4yrs and 5-11yrs. Dates for our Young People’s clubs (11-19yrs, up to 25 for young people with a special educational need or disability) are coming soon.  

We are open for referrals and enquiries for all of the Change4Life Service elements on offer. Partners, professionals or families can complete an online enquiry form via our website at: www.family-action.org.uk/Change4LifeService or contact us directly by phone. Download the enquiry forms for professional and self-referrals as Word documents if these would be more suitable.

Once an enquiry form is completed a member of the team will call to follow up and provide further information on the service that is being enquired about. 

Families that might be interested in participating in our clubs or coaching scheme can complete an enquiry form and find out more about the services on offer.
