Looked after Children's (LAC) Health Service

The service offers completion of the statutory review health assessments for all Looked After Children aged 5 years and over placed by Hammersmith & Fulham, Kensington and Chelsea and Westminster City Council.

The team support:

• the completion of a health care plan
• liaise with and support named social workers
• offer case working to support health promotion to children and young people not in school
• support foster carers to manage health conditions
• refer to community services as required
• follow up initial health care plan with multi-agencies as required
• complete a summary of health whilst in care to those rising 18 years of age
• support care leavers though a seamless transition to accessing of adult services
