A programme that can be implemented by clinicians which is designed to help patients and their families gain the knowledge and skills to manage their condition into adulthood. 

In healthcare, we use the word transition to describe the process of preparing, planning and moving from children’s to adult services.

We understand that for your patients, moving away from a team of doctors and nurses that they have been with for many years can be scary but hopefully, by getting involved in the transition process, you can empower them to feel more confident and happier about the move.

To support clinicians to deliver this this transition, we developed the Ready Steady Go transition programme.

The website also contains some useful easy-read docuements and information about the transition to adult services that can be used by young people and their families.
