Early Intervention and Support Team 0-5

This team will be the first service that families get referred to following a diagnosis and/or a referral from one of the Child Development Clinics.  The Practitioners in this team will be providing a higher level of support and working with families around targeted interventions identified at referral. Practitioners have had specific SEND training in the areas of: Portage, fussy eating, sleep routines, toileting alongside the usual support of parenting workshops and strategies to target specific behaviours. Portage will also be offered to children who are not in an educational setting between the ages of 0-3 years old.  

The aim of this team is to support families with navigating through universal and specialist services, to empower and equip families with understanding their child’s needs and building confidence with parenting a child with SEND.

Before your child reaches 6 years of age he/she will be assessed by the Early Intervention Service for next steps, this could be linking in with mainstream/inclusion services or a referral to the Bi-Borough Assessment and Reviewing Team.

Criteria for referral

Criteria children 0-5 being referred into Bi-borough Early Intervention and Support for Disabled Children’s Team: 

• Diagnosis of disability/complex health or physical disability needs
• Must be on the waiting list for a diagnostic assessment with CDS

AND

•   Significant delays of 12 months or more, in two or more areas

3. Language and Communication
4. Physical

How we will work with these children

• Cases discussed in Child Development Service (CDS) Referral Meeting, cases identified by CDS where targeted intervention is required to be referred into Early Intervention and Support Team.

• Referrals to be triaged by Disabled Children’s Service Management team

• Cases given to Service Navigator to complete initial assessment of family and child and provide recommendation as to what service is appropriate. Service Navigator will complete Initial Contact/Assessment.

Notes:

All children pre/post diagnosis will be able to access specialist universal support via workshops and programmes delivered by both Early Intervention and Support Team and Bi-Borough Inclusion Service.

Universal offer will be group workshops and coffee/drop in mornings. This includes workshops being offered by operations (sensory, communication, make and take etc)
