  • School Entry Health Assessment, covering areas such as height, weight, vision, hearing, continence, sleep, behaviour, and emotional wellbeing, to support a smooth and healthy start to school.

  • Clear handover from the nursery SENCo to the school SENCo, ensuring important information and support plans move with the child.

  • Quality First Teaching, with inclusive, high‑quality practice that meets the needs of all children in the classroom.

  • Early Help Best Start Family Hub transition support, offering advice and reassurance to help children and families feel confident about the move into school.

YES: Access to targeted services listed below:

  • Continued Special Educational Needs (SEN) support in school, ensuring children keep receiving the help they need as they move into their next stage of learning.

  • Ongoing APDR cycles (Assess, Plan, Do, Review) to regularly check progress and adapt support so it remains effective and appropriate.

  • Small group interventions and Teaching Assistant (TA) support, providing targeted help to build skills, confidence, and independence.

  • Educational Psychologist (EP) advice, where needed, to guide planning and support learning, behaviour, and emotional wellbeing.

  • An updated SEND Support Plan, clearly setting out current needs, targets, and strategies to support continued progress in school.


If difficulties are ongoing, your child could access specialist services such as:

  • Education, Health and Care Needs Assessment (EHCNA), if this has not already begun, to explore whether longer‑term statutory support is needed.

  • Education, Health and Care Needs Plan (EHCP) provision, where agreed, setting out the education, health, and care support your child will receive.

  • Embedded therapies such as Speech and Language Therapy (SALT), Occupational Therapy (OT), and Physiotherapy, delivered as part of everyday learning and routines where appropriate.

  • Ongoing support from community paediatric services, helping to monitor development and wellbeing over time.

  • Access to specialist placements, if needed, to ensure the most suitable learning environment for your child.

  • Support with formal Autism Spectrum Disorder (ASD) and Attention Deficit and Hyperactivity Disorder (ADHD) assessment pathways, including guidance and coordination with health and education professionals.
