This includes:

• child's details
• sibling details if you're also making a referral for a sibling group
• child's school or academy details
• parent or carer's details
• Penalty Notice request details

Documents you may need to upload

You may also need to upload the following:

• attendance certificate for current school year
• attendance certificate for the previous school year 
• parent's request for term time leave
• parent's notification of the Penalty Notice referral
• supporting evidence for unauthorised term-time leave referrals
• supporting evidence for persistent absence referrals
