Key Worker Service

About the service

We work with families to help them access support services for children with disabilities.

This includes:

  • providing emotional support
  • making referrals to other professionals
  • helping with benefit and other applications
  • liaising with other professionals
  • co-ordinating services

Eligibility

A child under the age of six is eligible if they show a delay in more than one area of development, which could significantly affect their everyday living. This could include:

  • global developmental delay
  • a learning disability
  • a sensory impairment
  • a physical impairment
  • chronic ill health or medical problems
  • a serious life limiting or terminal illness
  • a severe communication or language disorder
  • an autistic spectrum disorder with associated learning disability

Referrals

Referrals can be made:

  • directly by the family by phone or by email
  • by professionals involved with the family by completing a referral form - these are available from the Key Worker Service based at St Quintin Centre

Assessment

We will contact you to make an appointment for an assessment.
