Community Health and Wellbeing Worker Pilots in Golborne and Chelsea Riverside

We all have health and wellbeing needs, whether this be about our diets, fitness, social lives or mental health, and could benefit from the opportunity to talk to someone regularly about them.

Golborne Medical Centre in Golborne and Kings Road Medical Centre in Chelsea Riverside are working with The Royal Borough of Kensington and Chelsea Council to provide a team of Community Health and Wellbeing workers to residents in their areas. A Community Health and Wellbeing Worker is available to residents to visit their home on a monthly basis and provide support around whatever health and wellbeing needs the resident wants to talk about. They can also provide suggestions and connections to groups, organisations and services that could provide further help if needed.

If you have an assigned Community Health and Wellbeing Worker, you will receive a letter your GP practice to introduce them, and then they will be in touch to offer the opportunity for a full introduction and to start providing support around whatever you and your household needs around health and wellbeing. They will always have their council and NHS identification, and if you are not sure, you can contact your practice manager to confirm.

You do not lose your Community Health and Wellbeing Worker if you do not want to meet them straight away. They will stay in touch in a way that works for you, but you have no obligation to meet them, and when the time is right for you they will be ready to introduce themselves properly and support you as needed.
