Children with Disabilities Team

About the team

We provide advice, support and practical help to families with children up to the age of 18 years with a disability, who are substantially affected in their everyday living by a physical or mental impairment.

How we help

We will talk to you and your child to establish which services you may be eligible for. We may also, with your permission, liaise closely with other professionals such as:

  • teachers
  • health visitors
  • doctors

We work closely with parents. This includes assessing their needs as a carer.

We have close links with voluntary agencies involved with disability, such as Full of Life.

Before the young person turns 18, we make sure they have access to adult services if they need ongoing support.

We also work with the education service and transition workers to help the young person and the family prepare for adulthood.

How to access our services

We accept referrals from:

  • young people
  • their parents or carers
  • professionals

The best way to contact us is through our duty social worker who is available on 0207 598 4921 from 9am to 5pm.

If you can't visit the team because of your personal circumstances, we can see you at home or at another venue. 

Assessment

If we need to assess the child or young person, we'll make an appointment through a worker such as a:

  • social worker
  • social care worker
  • key worker
  • occupational therapist

Transition to adult services

We work closely with other services to help young people transition to adult services.

Transition planning starts at 16 years of age. To facilitate this:

  • a children and families’ worker works in the adult transitions team
  • an adult transition worker works in the children with disabilities social work team

The adult transitions team provides services up to the age of 25 years.
